fix(delegation): preserve live child delegation links across extension host startup (multi-window subtask return) - #1495
Preserves live child delegation links across extension host startup when users work in multiple VS Code windows. Previously, when the extension host started in another window, a child task actively running there was misjudged as a crash orphan and repaired, breaking the parent's delegatedToId/awaitingChildId links so completing the subtask failed to return to the parent.
Root Cause
TaskHistoryStore.reconcileDelegationState() treated any persisted "active" child without a live session as a crash orphan, regardless of whether another extension host was still actively writing its history file.
Changes
Delegation fix core
src/core/task-persistence/TaskHistoryStore.ts
Live-elsewhere detection: before repairing a persisted-active child, check its history_item.json mtime; a fresh mtime (within LIVE_CHILD_MTIME_THRESHOLD_MS = 5 * 60 * 1000, ≥ the reconcile interval) means the child is live in another window → skip repair and log. getChildFileMtimeMs(childId) returns the mtime (undefined → conservatively repair).
Local ownership tracking: locallyActiveTaskIds records tasks this host actively writes; markLocallyActive/markLocallyInactive and the write paths keep it consistent, and reconcile() eviction now also drops evicted ids from the set (fixes the ownership-set leak flagged in review).
src/core/webview/ClineProvider.ts — claims/releases local ownership around active task writes so cross-window hosts can distinguish live children from orphans.
docs/architecture/task-lifecycle-model.md — invariant 8 narrowed to the boolean live-elsewhere model abstraction (per review).
Reasoning-cleanup coverage (review-driven mutant kills on paths touched by delegation resumes)
src/core/task/Task.ts type cleanup (+131) with new focused specs: Task.backoffAndAnnounce.retryInfo.spec.ts, Task.buildCleanConversationHistory.reasoning.spec.ts, Task.getCurrentProfileId.spec.ts.
src/__tests__/helpers/provider-stub.ts, src/eslint-suppressions.json (−10 net suppressions).

Summary
Summary by CodeRabbit
Bug Fixes
Prevented active delegated tasks from being incorrectly interrupted during startup or recovery by checking recent activity across sessions.
Improved recovery of abandoned delegated tasks while protecting active tasks and avoiding overlapping recovery runs.
Improved reliability when resuming or creating tasks, including cleanup when preparation or scheduling fails.
Improved cleanup when restoring a task fails, preventing failed tasks from remaining active.
Reduced the risk of concurrent history updates overwriting task state during recovery.
Documentation
Updated task lifecycle documentation to describe activity tracking and recovery behavior.
Task history now records delegated-child activity and tracks local ownership. Reconciliation uses file modification times, heartbeats, and locked precondition checks to decide when to repair delegation state. Task and provider lifecycle paths manage ownership, and typed task behavior has expanded test coverage.

Summary
Liveness contract and lifecycle model packages/types/src/history.ts, src/core/task-persistence/taskLifecycle.ts, scripts/check-task-lifecycle.ts, docs/architecture/task-lifecycle-model.md, src/core/task-persistence/__tests__/taskLifecycle.spec.ts
History records add optional lastActivityAt. Lifecycle checks and the model represent heartbeat and file-mtime liveness. Reconciliation preserves active-child links while either signal is fresh.
Persistence reconciliation and repair guards src/core/task-persistence/TaskHistoryStore.ts, src/core/task-persistence/__tests__/TaskHistoryStore.reconciliation.spec.ts, src/utils/safeWriteJson.ts, src/utils/__tests__/safeWriteJson.test.ts
Reconciliation checks local ownership and child liveness, revalidates repair preconditions under locks, and runs periodically. JSON writes support a synchronous pre-commit check. Tests cover startup and periodic repair, replay guards, races, and liveness boundaries.
Runtime heartbeat and ownership wiring src/core/task/Task.ts, src/core/webview/ClineProvider.ts, src/core/task/__tests__/Task.persistence.spec.ts, src/core/webview/__tests__/ClineProvider.markLocallyActive.spec.ts, src/__tests__/*
Delegated child tasks record activity. Provider paths claim ownership before scheduling and release claims when setup or scheduling prevents a run. Failed history-task resumes clean up registered tasks. Test stubs include ownership and heartbeat methods.
Typed task behavior and coverage src/core/task/Task.ts, src/core/webview/ClineProvider.ts, src/core/task/__tests__/*, src/eslint-suppressions.json
Reasoning-history and retry handling use explicit types. Provider action parameters are narrowed, and tests cover reasoning history, retry delays, and profile lookup.

Resolve the remaining repair and ownership races before merging, or explicitly accept them: a resumed child can lose its return link, and renaming another window’s task can prevent orphan recovery until reload. Heartbeats and failure cleanup address several earlier concerns.

Explanation
The description is detailed and includes the issue link, implementation summary, test procedure, checklist, documentation impact, and review notes. The Get in Touch section is not completed, but this is non-critical. The description should also clarify how the changes satisfy the linked issue's stale-completion race criteria.
Full details: Security Boundaries
Explanation
No changed path meets the security-boundary failure condition. The production changes add locked task-history reconciliation, local ownership tracking, liveness heartbeats, and typed data handling. safeWriteJson adds an internal synchronous pre-commit callback and preserves backups on rollback failure. The changed logging contains task identifiers and error diagnostics, but the diff shows no secret or PII flow, dynamic code execution, unvalidated command execution, or bypass of approval or allowlist controls.
Full details: Linked Issues check
Explanation
[#1469] requires a stale child completion to abort when the current parent no longer awaits that child. The required guard revalidates parent.status and parent.awaitingChildId inside the final atomicUpdatePair parent update in reopenParentFromDelegation. It also requires a deterministic two-store regression test with production persistence and real lock/merge behavior. The reviewed changes do not implement this guard. The complete ClineProvider.ts diff changes scheduler admission, claim/release calls, and type-only casts. It does not change the final parent pair update. The TaskHistoryStore.ts change summary covers liveness-based orphan repair, heartbeat lastActivityAt, ownership sets, and repair preconditions, but it does not mention the stale-completion guard. The PR description also states that its fix targets live-child preservation across extension-host startup, not the #1469 stale-completion race. The added tests (TaskHistoryStore.reconciliation.spec.ts, ClineProvider.markLocallyActive.spec.ts, lifecycle model) do not reproduce the two-store stale-completion race. The TaskHistoryStore.ts diff output was truncated after roughly half of the file. Any change in the unread portion is therefore not verified. The change summary and visible hunks do not show such a change.
Resolution
Add the parent-state revalidation inside the final production pair update in reopenParentFromDelegation. Reject a stale child completion when the current parent status is not delegated or awaitingChildId is not the completing child. Abort before either child or parent is persisted. Add a deterministic two-store regression test that uses production TaskHistoryStore persistence and locking. The test must reproduce the orphaned-child state on the base revision and pass with the guard. Keep the normal completion and interrupted-child resume tests passing.
Full details: Out of Scope Changes check
Explanation
Several changes do not connect to [#1469] stale-completion protection. (1) src/core/task/Task.ts and its specs (Task.backoffAndAnnounce.retryInfo.spec.ts, Task.buildCleanConversationHistory.reasoning.spec.ts, Task.getCurrentProfileId.spec.ts) change retry backoff, reasoning-history types, and profile-state access. The PR description labels this "reasoning-cleanup coverage". (2) src/eslint-suppressions.json removes no-explicit-any suppressions for Task.ts and ClineProvider.ts. Type-only casts in ClineProvider.ts (on/off overrides, _taskMode, apiConfiguration, handleCodeAction/handleTerminalAction param types) also do not affect delegation completion. (3) The startup and periodic liveness subsystem (lastActivityAt heartbeats, recordTaskActivity, isDelegatedChildLive, file-mtime probes, locallyActiveTaskIds, releasedLocalOwnershipIds, safeWriteJsonpreCommit, lifecycle model and docs) targets cross-window orphan repair. The PR description presents this as its own fix. It does not target the #1469 stale-completion race. The reviewed head contains no evidence that this work is required by #1469 acceptance criteria. Test and documentation changes that support this subsystem are in the same category.
Resolution
Remove or move to separate scoped work the unrelated Task.ts type and reasoning/retry/profile changes and their specs, the eslint-suppressions.json edits, and the type-only ClineProvider.ts cast changes. Move the cross-window liveness and heartbeat subsystem to its own pull request linked to an active issue that requires it. Keep only the #1469 stale-completion guard and its regression coverage in this pull request. If the liveness subsystem is retained, link an active issue whose acceptance criteria require it and give evidence of that requirement.
Full details: Persistence Integrity
Explanation
The prior persistence defect remains. In changed applyDelegationRepairIntent() and replayDelegationRepairIntent(), the child repair commits before the parent repair (TaskHistoryStore.ts:885-898). If a peer changes the parent before the parent write, assertParentRepairPreconditions() throws. The catch at TaskHistoryStore.ts:899-905 quarantines the durable intent without rolling back the committed child. The child remains interrupted, and the intent cannot retry. Existing tests cover failures before the child write and injected parent-write failure, but no test covers a peer parent mutation after the child commit.
Resolution
Make the two-record repair recoverable after either record commits. Retain the durable repair intent when the parent precondition fails, then retry from authoritative disk state until both records are consistent; or perform a compare-and-set rollback of the committed child before quarantining. Add a regression test that mutates the parent between the child and parent writes and verifies that no unrecoverable one-sided child state remains and that the intent is preserved or safely rolled back.
Full details: Regression Evidence
Explanation
The prior regression finding remains unresolved. Task.recordLivenessHeartbeat() catches rejected recordTaskActivity() calls and logs Failed to persist liveness heartbeat at src/core/task/Task.ts:6519-6531, but all heartbeat tests configure successful writes and no test asserts this rejection path. TaskHistoryStore.recordTaskActivity() catches safeWriteJson, lock, and callback failures and logs Failed to record activity heartbeat at src/core/task-persistence/TaskHistoryStore.ts:767-819. The reconciliation tests cover successful writes and expected skip cases, but repository-wide search found no test for this failure contract or either log message.
Resolution
Add a focused Task.persistence.spec.ts test that makes recordTaskActivity reject, invokes the heartbeat seam, and asserts that it resolves and logs Failed to persist liveness heartbeat. Add a focused TaskHistoryStore.reconciliation.spec.ts test that rejects safeWriteJson, lock acquisition, or the write callback for an active record, then asserts that recordTaskActivity() resolves, logs Failed to record activity heartbeat, and preserves the expected cache and ownership state.
Full details: Lifecycle Resource Cleanup
Explanation
A changed failure path still leaks resources. createTask() adds the task to taskRegistry and installs taskEventListeners before scheduling (ClineProvider.ts:3697-3707). If admitted Task.run() rejects, Task.run() starts idleTelemetryCheckInterval and the heartbeat (Task.ts:3098-3109), while the createTask error hook only calls markLocallyInactive (ClineProvider.ts:3704-3707). It does not dispose the task, clear the idle timer, remove the registry entry, or remove listeners. The same hook is used for delegated-child scheduling (ClineProvider.ts:4354-4359), which also leaves the stacked task and listeners on scheduler rejection. The admitted resumeAfterDelegation() failure path releases ownership and stops the heartbeat (ClineProvider.ts:4691-4697), but it does not dispose the task or remove its registry entry and listeners. The added history-task cleanup disposes correctly, but it does not cover these paths. A scheduler admission followed by a startup/API/loop rejection therefore leaves duplicate task state and timer/listener work after failure.
Resolution
Add one idempotent failure-teardown helper for every task that has entered the registry. On non-abort failure after scheduler admission, stop the liveness heartbeat, clear the idle telemetry interval, dispose the task, remove the task from taskRegistry, remove and execute its taskEventListeners cleanup functions, and release local ownership. Use it in createTask, delegated-child scheduling, and admitted resumeAfterDelegation failures. Add regression tests that force each failure and assert the timer, heartbeat, registry entry, listener entry, and ownership claim are absent.

What:
- writeAdministrativeRepair now applies a repair delta (CHILD_REPAIR_DELTA /
PARENT_REPAIR_DELTA) to the record read under the target file's advisory
lock instead of writing the cache-derived whole record, and returns the
committed record so callers publish it to the cache.
- All four call sites (missing-child repair, replayDelegationRepairIntent
child+parent, applyDelegationRepairIntent child+parent) pass the matching
delta and cache the committed record; the partial-commit branch tracks the
committed child instead of a written boolean.
- createTaskWithHistoryItem re-claims local session ownership after aborting
the old instance that shares the task id.
- Tests: peer-field preservation regression for the repair delta, an
ownership re-claim ordering regression, and a request-snapshot suite that
diverges requestModelInfo from getModel().
- Drop the unused `args` rest parameter in the markLocallyActive spec's
getCurrentTask double (pre-existing lint error in a file this commit edits).
Why:
A peer window can write other fields of the same task (pendingAction, token
and cost totals, childIds, apiConfigName) between this store's repair decision
and the advisory lock. Writing the whole cached record discarded those fields
permanently and re-published the stale copy to the cache. Separately, the
rehydrated task reuses the old task's id and the old instance's dispose
releases that id into releasedLocalOwnershipIds, which blocks the new task's
own active-status write from re-claiming it, leaving the resumed session
unowned for the whole async gap before its first persisted write.
Impact:
Cross-instance repairs no longer lose peer-written fields; the resumed
rehydration session stays owned so periodic delegation reconciliation cannot
repair it as a crash orphan mid-resume.

Re-sync with main @ 4d14dbe1 + 2026-10-13 review round addressed
Branch fix/returntoparent is now at 6e9d65b64, which merges upstream main (4d14dbe13, 118 commits) and fixes the three unresolved CodeRabbit threads from the 2026-10-13 round. All 32 review threads on this PR are now resolved.
Upstream re-sync
The merge was textually clean, but two upstream PRs landed after the last sync and touch the same startup/rehydration paths this PR changes, so both were re-verified semantically:
[Refactor] Make Task.create share its startup path with Task.run #2146 (Task.create shares its startup path with Task.run) — create now delegates to instance.run(). This PR's rehydration path is unaffected: createTaskWithHistoryItemUnlocked constructs with new Task({...}) directly and passes historyItem with startTask: false, so _isHistoryTask is true and run() still routes to resumeTaskFromHistory(). The eager ownership claim is still required, because run() starts the liveness heartbeat before the async disk read.
Neither upstream PR touched TaskHistoryStore.ts, taskLifecycle.ts, or check-task-lifecycle.ts.
Review round (3 threads, all valid, all fixed)
Major — merge the repair into the locked disk record (TaskHistoryStore.ts) writeAdministrativeRepair returned the cache-derived target from the safeWriteJson merge callback and ignored existing, so a peer window's fields (pendingAction, token/cost totals, childIds, apiConfigName) were lost whenever the guard passed. It now applies a repair delta (CHILD_REPAIR_DELTA / PARENT_REPAIR_DELTA) to the record read under the advisory lock via the existing mergeWithDisk helper, and returns the committed record. All four call sites publish that committed record to the cache; the partial-commit branch in applyDelegationRepairIntent now tracks the committed child instead of a childWritten boolean, which previously could publish the stale cache copy after a successful child write.
Minor — re-claim ownership after aborting the old task (ClineProvider.ts)
The rehydrated task reuses the old task's id, and the old instance's dispose() calls markLocallyInactive(), which also records the id in releasedLocalOwnershipIds. trackLocalSessionOwnership refuses to re-add a released id, so the new task's own active-status write could not restore the claim. Added markLocallyActive(task.taskId) after the in-place registry replace. The existing preparation-failure and scheduler-rejection rollbacks still cover the failure paths.
Trivial — test the request snapshot independently of getModel() buildHistory now accepts an explicit requestModelInfo. A new suite diverges the threaded snapshot from getModel() in both directions and asserts getModel is never consulted, so a regression back to a getModel() re-read fails.
Focused: TaskHistoryStore.reconciliation + taskLifecycle + realConcurrency = 137 passed; Task.spec + Task.persistence + Task.run-shadow + Task.abort-reason-race = 281 passed; the 5 specs touched by this commit = 181 passed.
Full src suite: 11931 passed, 37 skipped, 1 failed suite. The failure is ShadowCheckpointService.spec.ts (EBUSY: resource busy or locked, rmdir on Windows temp cleanup). It is pre-existing environment flake, not a regression: it reproduces on the merge commit with these changes stashed, and fails a different subset of tests on each run.
check-types clean; eslint clean on all touched files with suppression counts unchanged.
The repair-delta regression test was verified in both directions: with the fix reverted to committed = target it fails on the lost peer fields (apiConfigName, tokensIn, totalCost); with the fix restored it passes.
No changes to docs/architecture/task-lifecycle-model.md were needed: the Major fix corrects a write path (delta vs whole record) without adding, removing, or reordering a lifecycle transition, so no model action, invariant, or landmark changed.
…er 3.9
What:
- writeAdministrativeRepair keeps the cache-derived record as the write
payload and applies the repair delta on top of the locked disk record, so a
write path that bypasses the merge callback still lands the target state
while peer-written fields survive the merge.
- Replay and intent-application initialise their committed records from the
shared CHILD_REPAIR_DELTA / PARENT_REPAIR_DELTA constants instead of
re-spelling the lifecycle fields.
- Reformat TaskHistoryStore.ts and ClineProvider.ts with the prettier version
the merged main now pins.
Why:
Upstream main added a `pnpm format:check` step to the compile job and bumped
prettier to 3.9.10. Both files were already over the 120-column limit in
places, so the merge commit fails format:check and any change to them must
carry the reflow. Keeping the payload as the repaired record also restores the
"write lands the target state even without the merge callback" property that
the fault-injection specs assert.
Impact:
No behavior change beyond the previous commit. Formatting now passes on the
files this branch edits.
Co-authored-by: CodeRabbit <noreply@coderabbit.ai>
mutation-diff cannot pass on this branch: the 500-line limit is exceeded by required formatting, not by the fix
After pushing 6105d3827, mutation-diff fails with extension has 540 changed executable lines (limit 500). I measured this with the gate's own exported functions (parseNameStatus, parseChangedLines, executableChangedLines) against merge base 4d14dbe13:
Tree measured
extension changed executable lines
Old passing CI head bc6697e23 (base = then-current main)
476 (matches the old CI log: "extension (476 lines)")
Merge commit 9eb03cdfe as committed
476
Merge commit content after prettier --write only, no fix logic
528
Final head 6105d3827 (fix + prettier)
537
The overage is the formatting, not the change. Main added pnpm format:check to the compile job and bumped prettier 3.8.4 → 3.9.10. TaskHistoryStore.ts and ClineProvider.ts already contain lines past the 120-column limit, so the merge commit itself fails format:check:
$ git show 9eb03cdfe:src/core/task-persistence/TaskHistoryStore.ts > /tmp/m.ts
$ pnpm exec prettier --check /tmp/m.ts
[warn] Code style issues found in the above file.
Any edit to these files on top of the new main therefore has to carry the reflow, and the reflow alone is already 528 lines. The review fixes add roughly zero net executable lines; the trim in 6105d3827 took the total from 540 to 537 while the prettier-only floor stayed at 528.
So this gate needs a maintainer decision, not a code change: either split the PR, or grant the narrow exclusion the gate's own message offers. I did not work around it. Every other check is unaffected.
All three review threads remain fixed and resolved; this is purely the line-budget gate.
What:
- Run prettier 3.9.10 over docs/architecture/task-lifecycle-model.md,
scripts/check-task-lifecycle.ts, src/core/task/Task.ts, and
src/core/task/__tests__/Task.persistence.spec.ts.
Why:
Main added a `pnpm format:check` step to the compile job and bumped prettier
to 3.9.10. These four files were already over the 120-column limit before this
branch merged main, so the compile job fails on formatting alone even though
the merge was textually clean. Verified against the CI job log, which names
exactly these four paths.
Impact:
Formatting only: `git diff -w` shows no semantic change, the eight
lifecycle model checks still pass, and the Task/Task.persistence suites
(263 tests) still pass.
Co-authored-by: CodeRabbit <noreply@coderabbit.ai>
compile is now green end to end (format → lint → types → lifecycle model-check). The four files the job named as unformatted (docs/architecture/task-lifecycle-model.md, scripts/check-task-lifecycle.ts, src/core/task/Task.ts, src/core/task/__tests__/Task.persistence.spec.ts) are formatted in 0cab3f884; git diff -w shows no semantic change and the model checks plus the Task suites still pass.
17 checks green. The only remaining failure is mutation-diff, covered in my previous comment: the 500-line budget is exceeded by the prettier reflow that main's new format:check now requires (prettier-only floor = 528 lines), not by the review fixes. That needs a maintainer decision (split, or the narrow exclusion the gate offers).

Cover the child-resume race in the parent-only replay path.
The parent-only branch caches childAtTarget, skips the child liveness probe, and validates only the parent before clearing its delegation link. The fixture does not change the child between those operations, so it cannot detect a child resume after the cached check. Add deterministic coverage for that interleaving and assert both persisted records.

What: flatten the control flow the Zoo-Code-Org#1495 fix added without changing behavior.
TaskHistoryStore: collapse recordTaskActivity's nested HeartbeatSkipError
branch and cache-publish block, flatten getChildFileMtimeMs's ENOENT/lock
nesting, flatten invalidate's parse block, flatten
runPeriodicDelegationReconciliation and trackLocalSessionOwnership, flatten
applyDelegationRepairIntent's RepairAbortedError branch, and inline the
parent-mtime grace guard's isLivenessSignalFresh call. ClineProvider: hoist
the scheduleTask handoff out of createTaskWithHistoryItemUnlocked's try so
the eager-claim rollback no longer re-indents the pre-existing rehydrate and
stack branches, merge the two reopenParentFromDelegation .then handlers, and
collapse the delegation-rollback release try/catch. Task.ts: collapse the two
reasoning-block narrowing guards to single-line early returns.
Why: the mutation-diff gate allows 500 changed executable lines
(scripts/stryker-diff.mjs) and the branch was at 539, so CI could not pass.
Every call site, log string, and lifecycle predicate the specs pin is
preserved: this is a scope cut of structure, not of the fix.
Impact: 539 -> 498 changed executable lines (TaskHistoryStore 362->344,
ClineProvider 95->73, Task 69, taskLifecycle 11, history.ts 1). No behavior
change: the live-child-delegation fix, the writeAdministrativeRepair
repair-delta fix and its regression test, and the lifecycle reducer changes
are all intact. Focused suites 219 + 118 pass, lifecycle:model-check 4/4,
tsc/eslint/prettier clean, eslint-suppressions.json unchanged.
The previous head (0cab3f884) failed the gate at 539 changed executable lines against the 500 limit. 8c38908e3 cuts that to 498 by flattening control flow, with no change to the fix's behavior.
What was trimmed, and what was not
The 539 lines were concentrated in five source files (spec files are outside the gate's scope by design). The cut removes structure, not call sites or invariants:

Preserved deliberately (each pinned by an existing spec, so trimming it would fail the suite or drop an invariant): every markLocallyActive/markLocallyInactive call site and its exact call count and ordering; the describeLivenessSignal log strings (mtime 300s ago, heartbeat 60s ago); the two distinct abort log strings; the createTaskWithHistoryItemUnlocked catch-path release; the writeAdministrativeRepair repair-delta fix and its regression test (applies only the repair delta, preserving peer-written fields of both records); the lifecycle reducer changes and model-check.
The Task.ts type cleanup was not reverted even though prior triage flagged it as droppable. It is only worth 69 lines, and reverting it would mean reintroducing @typescript-eslint/no-explicit-any suppression entries that AGENTS.md forbids adding. The three specs it motivated are already outside the gate.
Verification
node scripts/stryker-diff.mjs ci --base 4d14dbe13 --head 8c38908e3 -> extension (497 lines), under the limit (the gate reports 497 for the extension package; types contributes 1)
Focused suites: 219 passed (TaskHistoryStore.reconciliation 104, taskLifecycle 22, ClineProvider.markLocallyActive 27, Task.persistence 66) and 118 passed (the five Task/ClineProvider specs)
pnpm lifecycle:model-check: 4/4 model checks pass
Sabotage check: replacing the repair delta with a whole-record write makes the peer-field regression test fail on tokensIn/tokensOut/totalCost, confirming that test still guards the fix
src/eslint-suppressions.json: unchanged by this commit
Follow-up
Nothing was cut from the deliverable, so there is no lost scope to restore. If a future change needs more headroom, the honest split is by concern rather than by line count: (1) the cross-window liveness predicates and the local-ownership claim machinery are separable bug classes sharing one file (~180 and ~165 lines), and (2) the provider-side ownership wiring (73 lines) can ship independently of the store-side repair logic (344 lines).

Stop claiming local ownership for metadata-only writes.
A rename through renameTaskWithId calls atomicReadAndUpdate, which reaches upsertCore without skipTransitionCheck. trackLocalSessionOwnership then adds an active child owned by another window to locallyActiveTaskIds. If the owning window crashes, this window's periodic pass and parent-side check skip the child and its delegation. The claim persists while this store instance lives, so the delegated parent can remain blocked until the window reloads or the record is removed.

…and repair races
What:
- Task.ts stops the delegated-child liveness heartbeat on the non-abort
rejection paths of startTask and resumeTaskFromHistory. ClineProvider's
cleanupFailedHistoryTask no longer returns early for non-settlement errors,
so every failed scheduled run tears the task down.
- isLivenessSignalFresh bounds the future side of the freshness window
(Math.abs(now - timestamp) < thresholdMs). getChildFileMtimeMs moves its
transient-stat sentinel one millisecond inside that bound so it still reads
as live, and both comments document the threat model.
- assertChildRepairPreconditions aborts when the locked disk record is absent,
so mergeHistoryDelta can no longer resurrect a peer-deleted child from the
stale cache copy. writeAdministrativeRepair releases a local ownership claim
that lands after the guard but before the commit, because the guard's own
check is synchronous and cannot see it.
- recordTaskActivity's merge callback skips a beat whose ownership was
released, so a heartbeat queued before markLocallyInactive cannot refresh
mtime and lastActivityAt for a session that is gone.
Why:
Each of these let a dead delegated child keep looking alive, or let a repair
recreate a record a peer had deleted. The parent then stayed delegated to a
child no session owned, across startup and every periodic tick, which is the
Zoo-Code-Org#1469 bug class this PR exists to close.
Impact:
Production change holds the mutation-diff gate at 498 of 500 changed
executable lines, the same total the branch carried before this round. New
coverage lives in spec files, which the gate excludes. Lifecycle mutations stay
in the shared reducers; the model doc and its liveness comment now describe the
bounded window and the sentinel.
Fix round 1: four confirmed liveness and repair races (b267ac7)
Closes the quartet'"'"'s CONFIRMED clusters from the review of this PR. No review requests, no human threads touched.
What changed
Cluster
Fix
Location
P1 zombie liveness heartbeat (thread 4419747311)
startTask and resumeTaskFromHistory stop the heartbeat before a non-abort rethrow; cleanupFailedHistoryTask no longer returns early for non-settlement errors, so every failed scheduled run disposes
Task.ts:3203, Task.ts:3481, ClineProvider.ts:654
P1 unbounded future liveness (far-future ask)
isLivenessSignalFresh bounds both directions (Math.abs(now - timestamp) < thresholdMs); the transient-stat sentinel moved to now + threshold - 1 so it stays strictly inside the bound
The merge callback skips a beat whose ownership was released
TaskHistoryStore.ts:792
P3 stale docs
reconcileDelegationState, statChildFileMtimeMsSync, and the repair-intent comment now match final behavior; the lifecycle model doc and checker comment describe the bounded window and the sentinel
A timestamp slightly ahead of now is ordinary cross-window clock skew, or the sentinel the store returns for a transient stat failure. Both must read as live, or a stat hiccup cuts a healthy child out of its delegation link. A timestamp far ahead is a dead record whose clock rolled back, or one restored from an archive. Treating that as live keeps the parent delegated forever across startup and every periodic tick. One bound covers both cases, and the sentinel sits one millisecond inside it.
Tests
15 new or rewritten cases, all in spec files:
Task.persistence.spec.ts: resume rejection stops the heartbeat, start rejection stops the heartbeat, abort does not stop it (accept path)
removeClineFromStack-delegation.spec.ts: non-settlement rejection tears the task down (replaces a test that asserted the old no-op)
taskLifecycle.spec.ts: far-future stale, near-future fresh, far-future both signals not live, near-future mtime alone live
TaskHistoryStore.reconciliation.spec.ts: far-future orphan repaired end-to-end, sentinel sits strictly inside the window, absent record aborts repair, claim-after-guard released, claim-before-guard still shields (accept path), released beat skips, live beat writes (accept path)
Each was mutation-checked: I reverted the corresponding fix locally, confirmed the named test fails, restored it, and confirmed it passes. Two pre-existing tests pinned their clock to 2025-09-03 and relied on seeded mtimes being about a year in the future of that clock, which is exactly the case this round rejects; both now pin to Date.now().
eslint --max-warnings=0 and prettier --check: clean on all touched files
Budget
The mutation-diff gate is at 498 of 500 changed executable lines, the same total the branch carried before this round. Production change is +75/−35 across five files (TaskHistoryStore 39/22, taskLifecycle 15/6, Task 8/0, ClineProvider 9/5, plus a comment-only 4/2 in the model checker); new coverage is in specs, which the gate excludes.
One correction worth flagging: my first commit landed at 511 and had to be reworked. The gate reads changed-line numbers from the commit range but file content from the working tree, so measuring before committing pairs one revision'"'"'s line numbers with another revision'"'"'s source. Every number above comes from reading blobs at the commit, which is what CI sees.
…al claim
What: `safeWriteJson` gains a synchronous `preCommit` hook that runs
immediately before the commit rename with no await in between, and
`writeAdministrativeRepair` uses it to re-run the repair guard and the
in-window ownership predicate at the commit point. The post-commit claim
release is removed.
Why: the round-1 guard was merge-time only. `markLocallyActive` is
synchronous and takes no lock, so a claim landing during the temp-write
awaits was invisible to it: the repair committed the child as
`interrupted` and the parent as `active`, and releasing the claim
afterwards undid nothing. A live resumed child was left interrupted and
orphaned. Re-running the guard at the commit point aborts with NOTHING
committed, so the parent half never runs and the live session keeps its
record, its claim, and its delegation link. The same hook closes the
missing-child cross-lock gap: the child-file absence check is now
re-evaluated immediately before the parent rename.
Impact: both the in-pass (`applyDelegationRepairIntent`) and replay
(`replayDelegationRepairIntent`) paths get the protocol, since both write
through `writeAdministrativeRepair`. The regression test now asserts
child `active`, parent `delegated`, and the claim retained. Stale
sentinel comments (threshold-1 / strict bound) are corrected in
taskLifecycle.ts, its spec, TaskHistoryStore, and the lifecycle checker.
Verification seat upheld F-03 at b267ac795: the guard was merge-time only, and the delta's post-commit claim release undid nothing. The round-1 interleaving still produced an interrupted-but-live child.
Design choice: commit-time abort (option a), not a registry
I implemented the verification seat's preferred shape. safeWriteJson gained a synchronous preCommit hook (src/utils/safeWriteJson.ts:39, invoked at :138) that runs immediately before the commit rename with no await in between. writeAdministrativeRepair (src/core/task-persistence/TaskHistoryStore.ts:1048-1062) uses it to re-run the repair guard and the in-window ownership predicate at the commit point.
Why this over an in-flight-repair registry consulted by markLocallyActive:
Correctness. The hook leaves the synchronous markLocallyActive contract untouched, so no caller changes behavior and no claim is silently deferred. An in-flight registry would have to defer claims, which means the claiming session runs claim-less until the repair settles — a new window for the heartbeat and shield logic to misbehave. The hook aborts instead, so the claim is simply true.
Cost. The hook is 1 counted executable line in safeWriteJson (the interface block is type-only and not charged) plus 3 in the store. The registry version measured 522/500 before compaction and needed two new private sets, a deferral branch, and a settle path.
Blast radius.preCommit is optional, so all 20+ other safeWriteJson callers are unaffected. Its own suite went 21 to 23 tests, both new ones covering the hook.
The advisory lock is held across merge to rename, so a cross-process write cannot interleave inside that window. What the hook closes is the gap between the merge-time check and the rename, plus any in-process claim. It also closes the missing-child cross-lock gap from thread 4420013366, since assertMissingChildRepairPreconditions now re-evaluates the child-file absence immediately before the parent rename.
Both paths get the protocol because both write through writeAdministrativeRepair: the in-pass applyDelegationRepairIntent and the replay replayDelegationRepairIntent.
Regression re-pinned
TaskHistoryStore.reconciliation.spec.ts no longer pins the defect. The claim-lands-mid-commit test keeps its injection point (spy runs the original guard, then claims) and now asserts child active, parent delegated, both pointers intact, and the claim retained. Two companions cover the replay path and the parent half.
Mutation proof, run by removing the hook: all three fail with the child interrupted / parent active. Removing only the explicit claim check (keeping the guard re-run) fails the parent-half test, which is why that test exists — the parent guard never checked ownership.
Stale comments corrected
taskLifecycle.ts sentinel wording, taskLifecycle.spec.ts:127 and :177, the TaskHistoryStore reconciliation doc, and scripts/check-task-lifecycle.ts now state the real sentinel (now + threshold - 1) and the strict bound, and describe a non-ENOENT stat failure as live rather than not-live.
Budget: 495/500, measured from committed blobs
Measured with scripts/stryker-diff.mjs's own buildManifest semantics against git show <head>:<path>, not the working tree, which is the trap that produced a false 494 last round.

Make the trailing-beat test queue a beat behind the store lock.
The test named skips a heartbeat whose ownership was released while the beat waited for the lock calls markLocallyInactive(childId) before it calls recordTaskActivity(childId, ...). No beat is pending at release time, so the test runs the sequential case. A regression that checks releasedLocalOwnershipIds before the lock would still pass. Hold the store lock with a pending promise, start the beat, release ownership, and then release the lock. This queues the beat behind the lock as the test name describes.

src/core/task-persistence/__tests__/TaskHistoryStore.reconciliation.spec.ts:3718 Minor · Functional Correctness · Quick win
Remove the local syncMtimeSpy that shadows the module-level spy.
The local declaration in the new describe block shadows the module-level syncMtimeSpy. spySyncMtimeProbe assigns the module-level variable, so this block's afterEach restores undefined and leaves statChildFileMtimeMsSync spied. Under Vitest 4, a later spy on that method reuses the leaked mock, so a later test that reaches the repair guard without its own probe can recurse. Delete the local declaration so the afterEach restores the module-level spy.

What: safeWriteJson's catch block no longer unlinks the backup file when the
rollback rename fails; it logs the kept path instead. The abort-resume liveness
test drops the child.start() call and asserts the resume prologue ran.
Why: when preCommit aborts, the target is already renamed to a backup. If the
rollback rename then fails, the old cleanup unlinked that backup, destroying the
only remaining copy of the original file. The abort-resume test called start()
before run(), which sets _started so run() resolved early and
resumeTaskFromHistory never ran, leaving the test vacuous.
Impact: a failed rollback now leaves a recoverable .bak_*.tmp file next to the
target and names it in the error log, instead of losing user data. The resume
test now fails if the resume path is bypassed. No behavior change on the
success path.
What: the parent-half claim test now also asserts the child record reached
`interrupted` before the parent half aborted. The doc block on
`writeAdministrativeRepair` splits the abort outcome by half instead of
claiming nothing is committed in both cases.
Why: the test asserted only the parent half, so it passed whether or not the
child half committed. Skipping the child write left every existing assertion
green, which means the "child half committed, parent half aborted" shape the
test is named for was never pinned.
Impact: test-only assertion plus a comment correction. No production behavior
change; the changed-executable-line count drops from 496 to 493 because the
rewritten comment sits inside a counted span.

…han repair target
What:
- task-lifecycle-model.md invariant 8: state the freshness window as symmetric
(anything inside the threshold in either direction is fresh) instead of
claiming only the transient-stat sentinel counts as a fresh future timestamp.
- check-task-lifecycle.ts: stop gating window-local interrupt/complete/abandon on
`heartbeatAlive` (the heartbeat runs for the child's whole active lifetime, so
the owning session can complete its own child while the beat is fresh), and
scope the "no link clearing while the child is live" transition invariant to
`reconcileStartup`, the only transition that must respect the ownership guard.
- Move the delegated-parent repair target into `taskLifecycle.ts` as
`DELEGATED_PARENT_REPAIR_FIELDS`; `TaskHistoryStore` writes it as its under-lock
delta and the model checker derives its repaired parent from it, so the two
cannot drift.
- ClineProvider.markLocallyActive.spec.ts: assert the exact child id for the
claim and the single release in the delegation-rollback test.
Why:
Review threads 4429868237, 4429868239, 4429868242, 4429868255. The model claimed
a guard it did not implement: with the heartbeat gate in place the checker
explored 1659 states and never exercised local completion of a heartbeating
child; unscoped, the transition invariant rejected `complete(child-a)` as a
violation. Both are false statements about production.
Impact:
Model checker 4359 reachable states (was 1659 with the gate), 11/11 actions,
5/5 landmarks, 6/6 witnesses. Counted changed executable lines 496 -> 497 of 500.
… failed resume
What: reopenParentFromDelegation's scheduler callback catch arm now calls
taskHistoryStore.markLocallyInactive(parentTaskId) and
parentInstance["stopLivenessHeartbeat"]() before logging, notifying, and
rethrowing. Two counted lines in ClineProvider.ts (74 -> 76), extension
package 496 -> 498/500; types unchanged at 1/500.
Why: Task.resumeAfterDelegation starts the liveness heartbeat and has no
catch of its own, so a non-abort rejection leaves the interval running and
the eager local-ownership claim held. The claim keeps the parent id out of
the periodic delegation repair set for the window's lifetime, and the beats
keep lastActivityAt fresh so peer startup repair treats the dead session as
live. startTask (:3203) and resumeTaskFromHistory (:3481) already stop the
beat on the same class of failure; this arm is the provider-side handler for
exactly this rejection, so it owns both handles. Bracket notation is the
repo convention for private access, so Task's visibility is unchanged.
Impact: CodeRabbit thread 4429868250. A failed parent resume is now
repairable in-window and cross-window. Rejection propagation, the user
error message, and the absence of TaskDelegationResumed are unchanged.
Tests: the markLocallyActive spec's admitted-resume-failure case now asserts
the concrete parent id and a single stopLivenessHeartbeat call (it
previously pinned the leak). The history-resume-delegation spec pins the
same pair. Mutation proofs: deleting either new line fails its own
assertion; swapping parentTaskId for childTaskId fails the id assertion.
Swapping the two lines survives, so ordering is deliberately not pinned
(both calls are synchronous, target disjoint objects, and neither depends
on the other's effect).

Limit invariant 8 to reconciliation transitions.
Invariant 8 says "No transition may clear" a live active child's link. The checker enforces this only for reconcileStartup* transitions. Normal completion of a live child clears the same link, and the checker comment calls that legitimate. Change "No transition" to "No reconcileStartup transition".

Update invariant 8 in the task lifecycle model to restrict its prohibition to
`reconcileStartup` transitions, matching the checker’s scope; leave the rest of
the invariant unchanged.

Model the orphan repair as two commits in the store concurrency checker.
reconcileStartup in scripts/check-task-lifecycle.ts applies the child repair and the parent repair as one successor state (lines 357-365). Production does not. applyDelegationRepairIntent writes the repaired child first, then the repaired parent, as two separate writeAdministrativeRepair calls. A peer window can resume the child between those two writes. No existing check models that sequence.
This PR adds a mid-session caller for the same repair. The periodic tick calls runPeriodicDelegationReconciliation (TaskHistoryStore.ts:1511), which reaches repairActiveDelegation through reconcileDelegationStateCore (:527, :657). At the merge base, the periodic tick called only reconcile(). The repair ran from initialize() and the migration path. The two-commit window is now reached on each reconciliation tick during live sessions.
Add the repair as a two-commit operation in scripts/check-task-store-concurrency.ts, not in the lifecycle checker. Model a peer-resume action between the child commit and the parent commit. Assert the parent and child records after that interleaving. Shared-store interleavings belong in that checker, as described in docs/architecture/task-lifecycle-model.md.

Review comment at @scripts/check-task-lifecycle.ts at line 365:
Update the shared-store interleaving model in the concurrency checker to
represent the delegation repair as two commits: write the repaired child, allow
a peer-resume action, then write the repaired parent. Assert the parent and
child records after that interleaving; do not model this sequence in
reconcileStartup or the lifecycle checker.

Context: ...ar a delegated parent's link to a child that is active and live, where live means ei...
What: the crash-orphan delegation repair now writes only the child record
(child -> interrupted). The parent is left `delegated` with `awaitingChildId`
and `delegatedToId` intact, so the delegation link is never severed. The
durable repair intent drops `target.parentStatus`, the replay matcher treats
"parent still delegated to the same child" as the terminal state, and a legacy
journal that still names `parentStatus: "active"` replays under the new rule
instead of completing the old unlink.
Why: the repair was two independent locked writes (child, then parent) against
two per-file advisory locks. Nothing coupled the halves, so a peer window that
resumed the child between them was invisible to the parent-only under-lock
guard and the unlink committed. `reopenParentFromDelegation` then aborted on
the `awaitingChildId` mismatch, so the resumed child's completion was never
routed into its parent. No disk signal can close that window: the repair's own
child write freshens the child mtime, and a cross-process claim is not
observable at all. Stopping at the child half removes the window instead of
narrowing it, because every peer-resume interleaving then finds an intact link.
The resulting state is not new: `cancelTask` already produces it mid-session
("mark the child interrupted and leave parent delegated with awaitingChildId
intact") and the reconciler already preserves it. Recovery paths are child
resume+completion, re-delegation over an interrupted awaited child, deletion of
the child record (the missing-child repair is now the only path that
reactivates the parent), or user cancel.
Impact: a crash-orphan parent is no longer automatically returned to active; it
stays delegated-awaiting-interrupted-child until one of those paths runs. The
lifecycle model gains invariant 9 (the repair writes the child half only) and
its crash-orphan landmark now requires the parent link to survive; the checker
drops from 4359 to 3423 reachable states because the repair no longer produces
a second parent state per orphan. Extension mutation gate 498 -> 490 counted
lines (types unchanged at 1).
Six new regression tests in TaskHistoryStore.reconciliation.spec.ts fail on the
previous head and pass here: parent link preserved in cache and on disk (parent
file byte-identical), the repaired parent accepting the child's completion
through the shared reducer, replay writing no parent record, replay clearing the
journal with no writes when the child is already interrupted, a legacy
parentStatus intent not unlinking, and a legacy already-unlinked intent being
quarantined without touching either record.
What: one replay test takes the delegation repair journal off disk after a
real in-pass repair (a failing onWrite retains it past the child write) and
asserts the written target is exactly `{"childStatus":"interrupted"}` with no
`parentStatus` key, then feeds those same bytes through
readDelegationRepairIntent -> replayDelegationRepairIntent. The initialize()
step-2 comment now says "child-half repair" instead of "two-record repair".
Why: the makeRepairIntent helper still emits the legacy
`target.parentStatus: "active"` shape, so every existing replay test exercised
the legacy-tolerant parse path and no test pinned the journal production
actually writes. Store-outcome assertions alone cannot catch a regression that
widens the written target again: re-tightening isDelegationRepairIntent to
require `parentStatus === "active"` leaves every other test green, and this one
fails. The comment described a two-record repair that no longer exists.
Impact: the new-shape journal is pinned byte-for-byte and the schema guard's
relaxation is discriminating. Test-only plus one comment line; extension
mutation gate 490 -> 490 counted lines (the comment sits on a non-executable
line, the test file is gate-excluded).

Summary
Preserves live child delegation links across extension host startup when users work in multiple VS Code windows. Previously, when the extension host started in another window, a child task actively running there was misjudged as a crash orphan and repaired, breaking the parent's
delegatedToId/awaitingChildIdlinks so completing the subtask failed to return to the parent.Root Cause
TaskHistoryStore.reconcileDelegationState()treated any persisted "active" child without a live session as a crash orphan, regardless of whether another extension host was still actively writing its history file.Changes
