Fixes #2107

Problem

When the provider ends a stream mid-response (e.g. OpenRouter + Gemini: "Provider ended the request. Upstream idle timeout exceeded."), recursivelyMakeClineRequests pushed the same request back onto its stack with no upper bound. Without auto-approval the retry was immediate and silent (no backoff, no prompt); with auto-approval it backed off up to 10 minutes between attempts, indefinitely. A persistent upstream timeout therefore looped for hours.

Fix

Mid-stream failures now retry automatically at most MAX_MID_STREAM_AUTO_RETRIES (5) times, with the existing backoff, and only under auto-approval. Without auto-approval, or once the cap is reached, the user is asked via api_req_failed (same flow as the existing OutputTokenLimitError handling); confirming restarts with a fresh attempt count, declining stops the task.

Not covered

This does not fix the upstream timeout itself (provider pinning / reasoning effort remain the mitigations); it ensures the task stops looping and surfaces the error. Repetitive-reasoning detection is separate (#1904).

Tests

  • New it.each covering the auto-approval cap and the no-auto-approval prompt.
  • Existing tests that relied on the silent mid-stream retry now confirm the retry via a mocked ask.
  • core/task suites (922 tests) and check-types pass.

🤖 Generated with Claude Code

A mid-stream failure (e.g. OpenRouter 'Upstream idle timeout exceeded')
re-queued the same request with no limit. Without auto-approval it did so
immediately and silently, and with auto-approval it backed off indefinitely.
Retry automatically at most 5 times, then ask the user via api_req_failed.

📝 Summary

Summary by CodeRabbit

  • Bug Fixes

    • Automatic retries for mid-stream task failures are now limited to five attempts.
    • When automatic retry is disabled or the retry limit is reached, the task asks for confirmation before continuing.
    • Approving a retry resets the retry count.
    • Aborting during retry backoff continues to stop the task as expected.
  • Tests

    • Added coverage for retry limits, user-approved retries, persistent timeout failures, and abort behavior.

Task limits automatic retries for mid-stream failures to five attempts. When automatic approval is disabled or the limit is reached, Task asks the user before retrying. Tests cover the limit, backoff counts, and user-approved retries.

Changes: Mid-stream retry handling

Layer / File(s) Summary
Retry limit and approval flow
src/core/task/Task.ts
Task applies exponential backoff for up to five automatic retries. When automatic approval is disabled or the limit is reached, Task prompts the user and resets the retry count after approval.
Retry behavior coverage
src/core/task/__tests__/Task.spec.ts
Tests verify six total attempts and five backoffs for automatic retries. They also verify that manual retry mode prompts after one attempt and that existing retry scenarios confirm user approval.

Change: Bug fix · Severity of issue fixed: Medium

The retry cap and prompt work as intended. However, when a user confirms a retry after a mid-stream failure, the original request is saved to the conversation history a second time and sent to the model twice. This inflates context and can confuse the model on exactly the long-running tasks this change targets. Fix this before merging.

Full details: Description check

Explanation

The description identifies issue #2107, explains the problem, describes the retry cap and prompt flow, states the scope limits, and lists tests. It does not include the template checklist or several optional sections, but it contains the required implementation and validation details.

Full details: Out of Scope Changes check

Explanation

The PR changes only src/core/task/Task.ts and its focused Task.spec.ts tests. The retry cap, backoff, user prompt, fresh retry count, abort behavior, and test coverage directly support the linked stream-failure problem. No unrelated production behavior or unrelated test area is changed.

Full details: Security Boundaries

Explanation

No changed path meets a security failure condition. In src/core/task/Task.ts:4693-4712, mid-stream retries require current autoApprovalEnabled and stop at five automatic retries; the fallback uses ask("api_req_failed", ...), retries only for the exact yesButtonClicked response, and otherwise throws. api_req_failed is not a non-blocking or auto-approved ask (packages/types/src/message.ts:50-56,99-105; src/core/auto-approval/index.ts:194-199,427). The change does not execute input or alter tool allowlists.

Full details: Persistence Integrity

Explanation

No changed persistence path exists. The PR changes in-memory retry control in Task.recursivelyMakeClineRequests: it reads provider state, awaits ask, say, backoffAndAnnounce, and pushes retry items onto an in-memory stack. The changed tests only verify retry behavior. No write, default propagation, atomicity, rollback, or partial-failure path changed.

Full details: Lifecycle Resource Cleanup

Explanation

No changed lifecycle leak or post-cancellation duplicate work is present. The changed path returns the failed async generator before retry handling, and the existing finally clears isStreaming and currentRequestAbortController. Automatic retries await backoffAndAnnounce, which checks this.abort; the caller checks abort again before pushing the next request. The capped and prompted branches reset the retry item only after ask confirms, while the loop checks this.abort before starting another request. The existing ask cleanup also clears its timers on every settlement path.

Full details: Linked Issues check

Explanation

Issue #2107 requires the affected session to complete coding without the provider error. The changed Task.ts code does not fix the provider timeout. It still sends api_req_failed to the user after five automatic retries or when auto-approval is disabled, and it stops when the user declines. The change prevents an infinite retry loop, but it does not meet the issue's stated error-free completion outcome.

Resolution

Implement a provider or request-flow change that lets the affected Gemini/OpenRouter session complete without the timeout error, or revise the linked issue requirements if graceful failure and bounded retries are the intended outcome.

Full details: Regression Evidence

Explanation

The retry cap and prompt-decline paths have focused coverage in Task.spec.ts: auto-approval makes 6 attempts with 5 backoffs, and manual approval makes 1 attempt with one api_req_failed prompt (lines 626-664). The changed approval path at Task.ts:4700-4711 is not focused: the new test always returns noButtonClicked, so it does not verify that confirming at the cap retries or resets retryAttempt to 0. Existing output-token tests cover a separate branch and do not prove this mid-stream behavior.

Resolution

Add a focused mid-stream test that reaches the retry cap, returns yesButtonClicked from api_req_failed, and asserts the resumed request receives retryAttempt: 0 (and can complete or begin a fresh automatic retry window).

Assert that the declined retry rejects.

recursivelyMakeClineRequests throws new Error("API request failed") when the user declines, including after the automatic retry cap is reached. The current .catch(() => {}) hides this rejection. Assert it explicitly:

Assert each backoff attempt value.

A five-call assertion also passes if every retry uses attempt zero. The mutation-diff hint reports that this exact change to the argument at Task.ts Line 4721 survived. Assert attempts 0, 1, 2, 3, 4 for the auto-approved case, not only the call count. As per path instructions, require “behavior-focused assertions.”

Keep the history attempt separate from the automatic retry count.

On a confirmed retry, this stack item reuses currentUserContent but sets retryAttempt to zero. Line 4014 then passes the same retry state that added the first user message to shouldAddUserMessageToHistory. A non-empty request can append that user message again. Keep the history attempt positive while resetting a separate automatic-retry count. As per path instructions, check “persistence and lifecycle invariants” and “safe restart/resume without lost or duplicated state.”

The confirmed-retry stack item currently resets retryAttempt to zero, causing
shouldAddUserMessageToHistory to treat the reused currentUserContent as a new

while the separate automatic-retry counter is reset, preserving persistence and
