fix(apply-diff): fall back to a full-file search when :start_line: is stale - #2248
When apply_diff is given a :start_line: hint, the search is bounded to a window of bufferLines (40) around that line — including the line-number-stripping fallback. If the file grew since the line number was read, the hint can point hundreds of lines away from its target, so a SEARCH block that is present verbatim fails with a low similarity score. That is the symptom reported in #985.
This PR retries the match over the whole file before reporting failure, and accepts the result only when it is unambiguous.
findAllMatches(...) — scans the whole file and returns qualifying positions, stopping once the answer is decided. The existing fuzzySearch returns only the single best match, which cannot tell a unique match from a duplicated one.
resolveStaleHint(...) — tries the plain SEARCH content, then the line-number-stripped variant (mirroring the existing in-window aggressive fallback). It accepts a match only when exactly one position qualifies.
A new guard before the aggressive-strip block fires only when a hint was supplied and the in-window attempts failed.
The failure branch gains one diagnostic line when duplicates were found, so "the text exists twice" is no longer reported as "the text is absent".
Why the duplicate gate
The obvious risk of a fallback is silently editing the wrong occurrence. Rather than document that risk away, an ambiguous match is refused and the error names the number of matching places. A stale hint can therefore move an edit to its unique correct location, but can never pick between candidates.

Tests
src/core/diff/strategies/__tests__/multi-search-replace.spec.ts (+313) — 9 new cases in a stale :start_line: hint fallback (#985) block.
Six of them fail on the base file and pass with the fix:
Tests 6 failed | 84 passed (90) # base production file, new tests kept
applies the block at its real position when the SEARCH text is unique elsewhere in the file
refuses to guess when the SEARCH text matches several places in the file
reports the largest duplicate count across candidates, not the last one scanned
resolves a later block through the fallback when delta drift pushes its hint out of the window
honours a relaxed fuzzyThreshold in the full-file retry
keeps the verdict identical when the scan stops early on a long duplicate list
The other three are guards that pass on both sides, proving the fallback does not widen the matcher: absent text still fails, unhinted blocks are untouched, and multi-block line deltas stay correct when an earlier block grows the file. That last case pins the pre-existing delta arithmetic rather than the fallback: its adjusted hint lands exactly on its target, so the block matches in-window and the fallback gate stays closed. The fallback under delta drift is covered by resolves a later block through the fallback when delta drift pushes its hint out of the window, which fails on base.

Test Files 3 passed (3)
Tests 119 passed (119)
tsc --noEmit, eslint --max-warnings=0, and prettier --check are clean on both files. src/eslint-suppressions.json is unchanged.
Scope
Two files, no prompt or schema changes, no .changeset, no CHANGELOG edit. The diff is additive: the only modified existing line is the error string that now interpolates the duplicate hint.
Budget, measured from the committed blobs with scripts/stryker-diff.mjs's own selectFromGit() / executableChangedLines(): 65 changed executable lines of the 500 limit (one file, src/core/diff/strategies/multi-search-replace.ts).
… stale
What: when a hinted block fails inside the ±40-line window around its
:start_line:, retry the same match over the whole file before reporting
failure. The fallback accepts a match only when exactly one position
scores at or above diffFuzzyThreshold (verbatim by default), and reuses
the line-number-stripped variant as a second candidate. When several
positions qualify, the block still fails and the error now names the
duplicate count instead of implying the text is absent.
Why: a file that grows after the line number was read makes the hint
point hundreds of lines away from its target, so a verbatim SEARCH block
failed with a low similarity score even though the text was present
elsewhere. This is the reported Zoo-Code-Org#985 symptom, most visible when an agent
plans several large Markdown diffs from one read.
Impact: additive only, on the failure path of hinted blocks. Unhinted
blocks and blocks that match in-window are untouched. Refusing
duplicates means a stale hint can never silently edit another
occurrence.

Require regression coverage at the lowest valid harness with behavior-focused assertions, including relevant negative, error, false/unset, and boundary cases.
Edits with outdated line-number hints can find and replace a unique match elsewhere in the file, including when earlier edits have shifted line positions. Fuzzy matching requirements still apply.
Edits still fail when no match is found or multiple matches make the result ambiguous. A later unique match does not override ambiguity in another search candidate.
Ambiguity diagnostics report the largest duplicate count across candidates, indicating when the count is at least two if scanning stops early.
Edits are rejected when search and replacement content become identical after line-number hints are stripped.
When a hinted search has no match or falls below the configured threshold, the strategy retries across the full file. It applies a unique qualifying match and reports when multiple positions qualify. Regression tests cover retry outcomes, shifted hints, and related search behavior.

Summary
Full-file retry and candidate resolution src/core/diff/strategies/multi-search-replace.ts, src/core/diff/strategies/__tests__/multi-search-replace.spec.ts
The strategy scans the full file with the original search and, when applicable, its line-number-stripped form. It applies a result only when one position meets the configured threshold. Tests cover absent and duplicate matches, shifted hints, unhinted searches, and fuzzy matching.
Applying results and failure diagnostics src/core/diff/strategies/multi-search-replace.ts, src/core/diff/strategies/__tests__/multi-search-replace.spec.ts
The strategy uses a unique retry result in the existing application flow. It reports duplicate matches, including capped counts, and rejects search and replacement content that becomes identical after stripping. Tests cover these outcomes.

It does not reproduce the template headings or checklist, and it omits explicit documentation and contact sections, but the required change and test information are substantially complete.
Full details: Linked Issues check
Explanation
Issue #985 reports that a stale :start_line: hint makes an exact, present SEARCH block fail with a low similarity score. The issue proposes a full-file fallback, or at least a warning. The reviewed head meets this in multi-search-replace.ts. resolveStaleHint(...) runs only when startLine is set and the in-window match is absent or below fuzzyThreshold. It scans the whole file with findAllMatches(...), using the plain SEARCH and the line-number-stripped variant. It accepts a match only when exactly one position qualifies. Duplicates are refused, and the failure message adds a LIKELY CAUSE line that states the duplicate count. This addresses the misleading-diagnosis part of the issue. Absent text keeps the original error. The added tests in multi-search-replace.spec.ts cover unique recovery, duplicate refusal, absent text, relaxed threshold, and delta drift. The PR description and the previous assessment report the test results. This review did not rerun the test suite. The issue leaves the exact behavior open, so the unique-match design satisfies its stated expected behavior.
Full details: Regression Evidence
Explanation
Focused acceptance coverage now exercises the stale-hint stripped-candidate path. The added test at multi-search-replace.spec.ts:1800-1825 uses a stale hint outside the ±40-line window, has no literal 7 | ## Section B match, and verifies that the unique stripped ## Section B match is applied. The surrounding tests cover stripped-candidate ambiguity, absent text, duplicate diagnostics, no-op replacements, and unhinted behavior. The tests call applyDiff, so they do not merely mirror the private implementation. Prior failure addressed: The missing stripped-candidate acceptance case was added at multi-search-replace.spec.ts:1800-1825; it verifies a stale far-away hint, absent literal candidate, unique stripped candidate, and applied replacement.
Full details: Security Boundaries
Explanation
The changed path adds only bounded diff matching and replacement logic. The full-file retry does not emit file or search content, execute code, or alter approval or allowlist controls. It requires a qualifying match and rejects ambiguous matches before applying a replacement. No concrete secret/PII leak or security-boundary bypass is shown by the reviewed diff.
Full details: Persistence Integrity
Explanation
No changed persistence path exists. The pull request changes MultiSearchReplaceDiffStrategy.applyDiff and its tests only; the changed code searches and transforms in-memory resultLines, then returns content or failure diagnostics. It does not await, perform, or alter any file/database write, rollback path, or persisted default propagation.
Full details: Lifecycle Resource Cleanup
Explanation
No changed lifecycle resource path exists. The PR adds synchronous in-memory matching in resolveStaleHint and findAllMatches, plus replacement logic in applyDiff. The changed code does not create or register listeners, watchers, timers, tasks, subscriptions, or other disposable resources. It also adds no cancellation, disposal, or restart path that could duplicate work.
Full details: Out of Scope Changes check
Explanation
The stale-hint fallback, the duplicate diagnostic, and their tests connect to #985. The second identical-content guard does not. It is added after the aggressive stripping block in applyDiff (the block labelled Stripping above can make SEARCH and REPLACE identical...). That stripping block runs for every block that fails the in-window match, with or without :start_line:. The guard therefore changes behavior for unhinted blocks. A block whose SEARCH and REPLACE differ raw but become identical after stripping now returns success: false with Search and replace content are identical. Before this change, the same block matched and wrote the file back unchanged. The PR description states that unhinted blocks are unchanged and that the only modified existing line is the error string. Both statements are inaccurate for this guard. The commit fix(apply-diff): refuse ambiguous stale-hint candidates and no-op replacements describes this guard as a separate fix. The guard is not required by #985, and no test in the PR description covers it.
Resolution
Remove the post-stripping identical-content guard from this PR. Keep only the existing pre-change identical check and the #985 changes. If the no-op rejection is still wanted, move it to a separate PR with its own issue, tests for the unhinted path, and a matching PR description entry. Correct the PR description so that it states every behavior change to unhinted blocks, and so that the modified-line claim is accurate.

Add a stale-hint test for a numbered SEARCH that matches only after stripping.
When the REPLACE section has no line numbers, applyDiff does not strip a numbered SEARCH before the stale-hint retry. The retry then reaches the stripped candidate in resolveStaleHint, and that candidate can match outside the ±40-line window. The six added tests use plain SEARCH text, so no test covers this branch. Removing the stripped candidate would leave them passing. If both sections are numbered, the SEARCH is stripped earlier, so the test must use a plain REPLACE section.

…st duplicate count
What: findAllMatches now stops at the fourth qualifying position and reports
whether it truncated, because the caller only needs to tell 0 / 1 / >=2
apart. resolveStaleHint keeps the maximum count across candidates instead of
the last one scanned, and the duplicate diagnostic prints "at least 2" when
the count is a lower bound instead of a wrong number.
Why: the full-file retry scanned every position even after the verdict was
decided, which is a needless Levenshtein sweep on large files. Separately, a
SEARCH block whose plain text matches three places and whose stripped form
matches two reported "2", understating the ambiguity the message exists to
explain.
Impact: the accept path is unchanged, so an exact-one match still applies at
the same position. Only the failure diagnostic changes, and only for counts
past the cap. Two new tests pin the early exit (ambiguous stays ambiguous, a
unique match still applies) and the maximum-count rule; the multi-block delta
guard is reworded to say it pins in-window alignment, with a new case that
drives the fallback through delta drift.
…lacements
What:
- resolveStaleHint no longer accepts a single hit from the line-number-stripped
candidate once the literal SEARCH text has already matched several places.
The block is refused and the largest count found is reported.
- applyDiff re-checks SEARCH/REPLACE equality after stale-hint and aggressive
stripping. A pair that differed raw but collapses to identical content now
reports a failed part instead of succeeding with the file unchanged.
- Correct the MAX_MATCHES comment to the string the code renders ("at least 2"),
fix the delta-drift comment arithmetic (199 growth, 119 offset), and pin the
scan cap from both sides with an exactly-4 fixture.
Why:
- The stripped candidate could match exactly one line while the literal text
matched two, so the fallback edited a line the model never pointed at and
reported success.
- `| foo` -> `foo` strips to identical content on both sides, which made
applyDiff return success with byte-identical output.
- The cap boundary had no exactly-4 fixture, so `===` -> `>` survived all 119
pre-existing tests.
Impact:
- Ambiguous stale hints now fail loudly with a duplicate-count diagnostic
instead of guessing. No-op blocks fail instead of silently succeeding.
Unique and stripped-only matches keep their previous behavior.
Tests: 119 -> 124 in core/diff (3 regression tests fail on the pre-fix code;
the `>` cap mutant is killed by the new boundary test and survives all 119
pre-existing tests on unmodified HEAD).

Summary
When
apply_diffis given a:start_line:hint, the search is bounded to a window ofbufferLines(40) around that line — including the line-number-stripping fallback. If the file grew since the line number was read, the hint can point hundreds of lines away from its target, so a SEARCH block that is present verbatim fails with a low similarity score. That is the symptom reported in #985.This PR retries the match over the whole file before reporting failure, and accepts the result only when it is unambiguous.
What changed
src/core/diff/strategies/multi-search-replace.ts(+133 / -1):findAllMatches(...)— scans the whole file and returns qualifying positions, stopping once the answer is decided. The existingfuzzySearchreturns only the single best match, which cannot tell a unique match from a duplicated one.resolveStaleHint(...)— tries the plain SEARCH content, then the line-number-stripped variant (mirroring the existing in-window aggressive fallback). It accepts a match only when exactly one position qualifies. Why the duplicate gate
The obvious risk of a fallback is silently editing the wrong occurrence. Rather than document that risk away, an ambiguous match is refused and the error names the number of matching places. A stale hint can therefore move an edit to its unique correct location, but can never pick between candidates.
