The documentation was built around a frameset, which is obsolete HTML
and which didn't work well on boost.org.  The `<noframes>` fallback was
no help either, as its markup was malformed and produced no link at all.

The contents are now the documentation home page, contents.html
redirects there so that existing links keep working, and the `target`
attributes which addressed the frames are gone, so every link navigates
in place.  The title of each page links back to the contents, which the
frame used to keep in view.

Fixes #300.
The last branch of `initialize()` tested `navigator.appnName`, which
does not exist and would have thrown had it been reached.  It is not
reached in practice, since every current browser reports `appName` as
"Netscape" and the first branch always wins, but the check was wrong
just the same.
