docs: allineare README, man page e guida a comportamento recente

  • ~/.cache/pcm: helper senza segreto (password via ambiente), file VNC/SFTP e sweep all'avvio\n- conservazione di known_hosts, export senza TOTP a 0600, RDP/VNC fuori da argv\n- man page: env var corretta (PCM_ASKPASS_PW) + PEM_KEYPASS_PW/PCM_RDP_PW\n- guida HTML: cifratura include il TOTP, chiarito il plaintext opzionale, path log corretto\n- README: sezione Sviluppo (pytest + ruff)
