Ask a wrapping-up agent to close the loose ends it finished - #1932
Agents keep reporting loose ends that their own session already finished hours ago, because nothing at wrap-up asks them to close those ends. The Stop-hook block gets a new first step: close_loose_end each loose end whose work this session finished, the ones it declared (by their declaration_id, or among the ends list_loose_ends returns for session_id, which lists what the session sighted and so includes what it declared) and the ones it claimed (found by claimed_session_id, each closed with its claim_id). Declaring and next work become steps 2 and 3. The completion instruction gets the same step, so the session-start guidance and the workitems MCP server's instructions agree with the Stop hook
Where to look
The MCP server's instructions start with the completion instruction, and Claude Code keeps only their first 2,048 characters, which McpServerInstructionsTests pins. The clause there is the short form ("close_loose_end what you finished, declare the rest"), which keeps them at 2,044
The agent now closes loose ends it completed during a session before reporting unfinished items or checking for next work. This applies to both items it declared and items it claimed, helping keep completion status current and preventing finished work from remaining listed as outstanding. Items still unfinished are reported after completed items are closed.
The completion instruction also leads the workitems MCP server's instructions, which Claude Code cuts at 2,048 characters; the longer text still fits

The completion nudge and next-work instruction tell the agent to close completed loose ends before declaring remaining ones. Tests check the guidance and confirm that get_next_work follows the close and declare steps.
The SessionStart completion path may leave finished loose ends open and cause them to reappear in later sessions.
Security architecture risk: Moderate · up to 5fa7b
The new guidance helps close finished work, but one suggested lookup can also return work the session merely saw. Existing instructions distinguish that from ownership; the final enforcement of close requests could not be verified.
Retained concerns
Low · security · inferred: The new fallback for finding a declared loose end uses a session-sighting filter, not a declaration or ownership filter. If a sighted item is mistaken for one this session finished, the guidance can lead to an unclaimed close request; whether the service rejects a close that conflicts with another worker's claim is unverified.
Security review details
Security Blast Radius
inferred — The changed instructions can cause additional close requests against loose ends visible to the working session. The inspected client defaults listing to the current repository but accepts an explicit repository scope; the service's ultimate user and repository authorization is not established here.
Security Findings and Attack Paths
inferred — No unauthorized close is verified. The plausible integrity path is selection of a merely sighted loose end from the new session_id lookup, followed by a close without a claim ID; the service's handling of an item claimed by another worker remains unknown.
Trust Boundaries and Controls
observed — The guidance expressly separates declared from claimed work. For claimed work, the client sends claim and working-session identifiers to the completion endpoint and rejects acknowledgements that do not match them; these controls do not prove what the remote service accepts.
Resilience and Maintainability Implications
observed — On an unconfirmed claimed completion, the client directs inspection before retrying. The existing unclaimed close path can retry without an inferred session after session_not_found; neither behavior was introduced by this PR.
Hardening Proposals
proposed — Make declaration ID or verified item identity the preferred basis for a declared close, and clarify that a session_id result is only a discovery aid, not proof of declaration or ownership. Verify service-side active-claim fencing and retry semantics before relying on multi-item close guidance.

Explanation
Issue #1788 asks that the Stop-hook nudge tell the agent to verify and close loose ends. The reviewed head changes the BlockDecision reason in ClaudeCompletionNudge.cs. Step (1) now tells the agent to call close_loose_end for each loose end whose work this session finished. Declared ends are identified by declaration_id, or through list_loose_ends by session_id. Claimed ends are identified through list_loose_ends by claimed_session_id, and each close passes its claim_id. The 'whose work this session finished' condition gives the verification step that the issue title asks for. Declaring remaining ends and get_next_work are now steps (2) and (3). NextWorkEmitter.CompletionInstruction in NextWorkEmitter.cs adds the same close step, so session-start guidance matches the Stop hook. The Stop-hook text names session_id only for declared ends. The PR objectives say session_id is excluded, but that exclusion applies to claimed ends, and the code does not use session_id for claims. This is consistent. The unit tests that pin the wording (ClaudeCompletionNudgeTests, NextWorkEmitterTests) were not re-executed in this review. The inspected diff matches the PR description. The pinned wording is verified from source only.
Full details: Out of Scope Changes check
Explanation
The whole-PR diff from merge base f130862 to head 5fa7b76 changes three areas. ClaudeCompletionNudge.cs changes the BlockDecision reason text and its XML summary. NextWorkEmitter.cs changes the CompletionInstruction text. The unit tests pin the new wording. Every inspected hunk adds or reorders the close_loose_end step, which issue #1788 requires. The CompletionInstruction hunk uses the shorter clause described in the PR description, to keep the MCP server instructions within the 2,048-character limit. No unrelated behavior change appears in the inspected hunks. The PR description reports 2,044 characters and 155/155 passing tests. The PR objectives give different figures, but these do not affect scope.

High-Level Assessment
Keep the close step in both instruction surfaces: the Stop hook and the shared completion instruction reach agents at different times, while the latter also supplies MCP server guidance. Centralizing the text in one place would require additional wiring for little benefit to this focused change.

Description
The Stop-hook instruction treats list_loose_ends with session_id as finding loose ends this
session declared, although that filter returns every end this session merely sighted. When another
session's claimed work appears in this session's feed, the agent can select its ID and attempt to
close work it neither declared nor owns.

Accepted precedent supports correcting session-scoped completion guidance; ownership semantics make
this a concrete correctness issue.
ⓘ Recommendations generated based on similar findings in past PRs
Evidence
The work-items guidance explicitly says claimed_session_id finds ownership while session_id only
filters where work was sighted. The MCP server forwards these as independent query parameters, and
claimed completion requires the current claim and working session, so the new instruction can select
unrelated rows and cannot safely treat them as this session's declared or owned work.
The issue below was found during a code review. Follow the provided context and guidance below and implement a solution
### Issue description
The new close step says `session_id` or `claimed_session_id` finds loose ends this session declared or claimed, but `session_id` filters where an end was sighted rather than who declared or owns it. This can expose another session's work to the wrapping-up agent and allow an incorrect close attempt.

### Recommended Fix
Rewrite both instruction surfaces to distinguish declared ends from claimed ends. Use only evidence that the current session declared an end, and use `claimed_session_id` plus the current claim metadata for work owned by this session; do not describe `session_id` as an ownership or declaration filter. Preserve the requirement to pass the current `claim_id` when closing claimed work, and add a regression test asserting that the generated instructions do not equate `session_id` with ownership.

Closes #1788 — AI-4171
What & why
Agents keep reporting loose ends that their own session already finished hours ago, because nothing at wrap-up asks them to close those ends. The Stop-hook block gets a new first step:
close_loose_endeach loose end whose work this session finished, the ones it declared (by theirdeclaration_id, or among the endslist_loose_endsreturns forsession_id, which lists what the session sighted and so includes what it declared) and the ones it claimed (found byclaimed_session_id, each closed with itsclaim_id). Declaring and next work become steps 2 and 3. The completion instruction gets the same step, so the session-start guidance and the workitems MCP server's instructions agree with the Stop hookWhere to look
The MCP server's instructions start with the completion instruction, and Claude Code keeps only their first 2,048 characters, which
McpServerInstructionsTestspins. The clause there is the short form ("close_loose_end what you finished, declare the rest"), which keeps them at 2,044Verification
